For Teenagers


AHP recognizes that teens have special rights of privacy concerning their medical history and records.  Because of this, the record of a patient 14 or older will not be visible unless the patient has formally requested portal access. 

Teen patients may choose to make their records visible under the parent’s portal account (as they are today), or may choose to have their own portal account with separate login. Health records of teens will only be visible on the portal if they make one of these choices.

Each teen will need to individually request that their health records be accessible on the portal. All requests require the signature of the teen patient; parents may not make this request on behalf of their children.

The Teen Portal Request should be used by teens to request portal access to their health records.
